Health First Network and MED3OOO Proactively Manage Patient Care Through Population Health Management Initiatives

PITTSBURGH, April 19, 2007 — MED3OOO, Inc., a national healthcare management and technology company, recently purchased the Trizetto Facets e2 enterprise software and Clinical CareAdvance(TM) care management module to enhance population health management initiatives with its IPA and health plan clients. MED3OOO will be deploying these initially with Health First Network in Pensacola, FL. The Population Health Management Initiatives will help the IPA to identify patients at risk for advanced disease and multiple morbid conditions, improving the patient's health status and adherence to standards of care while reducing healthcare expenses and further improving the quality of care delivered in the network.

Health First Network (HFN) is committed to managing the health of the population they serve by providing the right intervention(s) to the right patients at the right time, and MED3OOO is providing the management and technology for this proactive approach to healthcare. MED3OOO has designed a Population Health Management program for HFN that includes disease management, case management, and, most recently, predictive modeling. Through tools like MEDai's medical intelligence, which uses predictive modeling to identify patients at risk, and Trizetto's Clinical CareAdvance software to help better manage and coordinate the patient care, the IPA expects to see significant results.

Health First Network has begun using the MEDai predictive modeling software and will now be better able to intervene and coordinate care with the new integrated Clinic Care/Advance component for their Population Health Management program.
