Dr. Jacqueline Delmont is a Board Certified Internist who has built a remarkable career serving low-income and underprivileged patient populations across Long Island and Queens. Through her own efforts, hard work, and the force of her personality, Dr. Delmont was able to help thousands of men, women and children to obtain the medical care they need and dramatically improve their lives. As a newly-appointed Medical Director of Beacon Health Partners, a Long Island based accountable care organization recently selected to Participate in the Medicare Shared Savings Program, Dr. Delmont is responsible for all care coordination, disease management, and physician leadership activities. In addition, she is in charge of establishing and implementing clinical guidelines and patient-centered medical home transformation. In 1999 Dr. Delmont established her private practice in the Freeport-Roosevelt area, known as a federally-designated healthcare professional shortage area. In 2005, Dr. Delmont became the Operator and Medical Director of a failing Diagnostic and Treatment Facility (Primary Healthcare Plus) in Franklin Square, transforming the organization into a thriving treatment facility, delivering efficient and high quality care to thousands of low-income, uninsured and underinsured patients. In 2007, Dr. Delmont's team was hired by Backstretch Employee Service Team (B.E.S.T.) - a local advocacy group for racetrack employees. The healthcare program was restructured, culturally sensitive medical care was provided to the predominantly Latino workers and as a result of this initiative, utilization of preventive services tripled in two years. Dr. Delmont's medical team works closely with major minority-led not-for-profit organizations. In addition, her group represents minorities in clinical trials, researching new medications for the chronic conditions that are so prevalent among blacks and Hispanics. Besides her extraordinary medical career, Dr. Delmont has established herself as a prolific lecturer and speaker, sharing her insights on a wide variety of health-related, women-related and community issues. In addition, she frequently appears on popular television programs, rendering her expertise to minority communities nationwide. Dr. Delmont was a recipient of the prestigious 2008 Women of Distinction Award and the 2009 Hispanic Heritage Award. Recently, she has been acknowledged with the Physician Hero category of the annual Healthcare Hero awards, sponsored by Long Island Business News. Dr. Delmont became Board Certified in Internal Medicine both in her native Venezuela in 1995 and the United States in 1999. Currently she is working towards her master's degree in Business Administration at Adelphi University.
